Severe heart failure — NSAIDs should be avoided.
COX-2 inhibitors, diclofenac, and high-dose ibuprofen (2400 mg or more daily) should be avoided.
Diclofenac, etodolac, meloxicam, and nabumetone, are also nonselective NSAIDs, but are thought to have a preference for COX-2.

In patients without established cardiovascular disease, avoid initiation if eGFR less than 60 mL
with established cardiovascular disease, avoid if eGFR less than 30 mL/minute/1.73 m2.
Limit dose to 10 mg once daily if eGFR less than 60 mL/minute/1.73 m2;
